Sparta Prague won their seventh Czech title in a decade by thrashing Most 5-0 on Monday.
Sparta, with 34 league titles in total, also gained their first double in 18 years thanks to a 2-1 win over Jablonec in the Czech Cup final last Thursday.
In a tight finish, the top four spots were up for grabs until the last round of the championship.
Title defenders Slovan Liberec, who had trailed Sparta by just one point before the last round, lost 4-0 at Mlada Boleslav and finished fourth behind all their rivals.
Slavia Prague sealed second place with a 2-1 victory at Ceske Budejovice, thanks to better head to head results than Boleslav and Liberec after all three teams finished with 58 points, four behind the champions.
Sparta will enter the Champions League at the third qualifying round, while Slavia go into the second qualifying stage. Boleslav earned a spot in the first round of the UEFA Cup.
